Required Documents

To complete the application form, you will need to upload the following documents so have them prepared beforehand. Document scans should be legible, and uploaded under the appropriate sections.

  1. Proof of Payment for the Application fee of US$50.00 is required.
  2. Learner’s Birth Certificate.
  3. Learner’s Latest two  academic Term Report.
  4. Latest Sports and Arts achievements.
  5. Learner’s Baptism Certificate or Equivalent.
  6. Learner’s Recommendation Letter from current school Head.
  7. Learner’s Recommendation letter from Minister of Religion.
  8. Learner’s Passport Photo.
If you are applying for more than one child/ward, you can do another application after completing the first one. Current parent should refer to their D6 Connect profile and check their family code if they are applying for a current learner’s sibling.
